Respect

Proficient plus makes extra effort to assist others, always courteous, helpful and pleasant partner.

Generally courteous to coworkers and supervisors.  Cooperative partner.

Shows courtesy to coworkers and supervisors most of the time.  Occasionally interferes with the learning, safety or well being.

Impatient with coworkers.  Little evidence of common courtesy.  Often interferes with the learning, safety or well being of others.

Productivity

Proficient, plus worker gives extra time or effort and produces exceptional products.

On task during work time, product is produces efficiently and is well done.

Works most of the time, some progress on product, work may be messy or partially done.

Wastes lots of work time.  Little accomplished during work hours.

Initiative

Proficient, plus assesses situations and takes necessary actions on own.  Leads others towards successful attainment of goals.

Shares new ideas and organized action.  Regularly volunteers for chores.

Must be asked to participate in tasks or help with chores.  Helps willingly and shares some opinions and ideas when requested.

Must be asked to participate or help with chores.  Reluctant to help and withholds needed ideas.

Integrity

Proficient, plus exemplifies integrity by leading coworkers to value honesty.

Work is obviously own, data is accurate, and is truthful about activities.

May have copied others’ work, falsified data, or lied about activities, but chooses to discuss these faults privately with supervisor.

Copies others’ work, or falsifies data, or lies about activities and does not admit to these faults.

Dependability

Proficient, plus volunteers to take on responsibility and can be depended on to follow through.

Can be depended on to follow through with most activities on own.  Uses agenda to remind self of assignments and other obligations.

Follows through with promised activities with reminders.  Irregular use of agenda.

Cannot be depended on to follow through with promised activities.  No use of agenda.

Accuracy

Proficient, plus 90% or more of the knowledge or numbers are correct.  Shows excellent understanding of the entire job.

Work is written clearly, and 80% or more of the knowledge or numbers are correct.  Fulfills all parts of instruction.

Work is difficult to read, or less than 79% of knowledge or numbers are wrong, shows partial understanding of job.  Fulfills part of instructions.

Work is illegible, or knowledge or numbers are wrong or missing, shows not clear understanding of job.  Ignores many parts of the instruction.
